When evaluating a surgeon, complication rates are among the most cited quality metrics. But raw complication rates can be deeply misleading. A risk-adjusted complication rate is the metric that actually reflects surgeon quality.

What Is a Risk-Adjusted Complication Rate?

A risk-adjusted complication rate accounts for patient health characteristics before comparing outcomes between surgeons. It answers: given the complexity of this surgeon's patients, how did their actual complication rate compare to what was statistically expected? A rate below 1.0 means performing better than expected. Above 1.0 may indicate a quality concern.

Why Raw Complication Rates Are Misleading

A surgeon treating only low-risk patients will have a lower raw complication rate than one accepting complex cases — even if the complex-case surgeon is delivering superior care. Risk adjustment removes this distortion. Surgeon A with a 6% raw rate treating high-complexity patients may be outperforming Surgeon B with a 3% raw rate treating healthy patients.

How Risk Adjustment Works

Risk adjustment models use statistical regression to predict the expected complication rate based on age, BMI, diabetes status, cardiovascular history, kidney function, prior surgeries, and procedure complexity. The surgeon's actual rate is compared to the prediction, producing an observed-to-expected (O/E) ratio. An O/E of 0.75 means 25% fewer complications than expected — a strong signal. An O/E of 1.30 means 30% more than expected.

Society of Thoracic Surgeons (STS) — For cardiac surgery including CABG. STS star ratings (1–3 stars) are derived from risk-adjusted mortality, complication, and reoperation metrics. The most robust publicly accessible risk-adjusted registry for cardiac surgery in the US.
ACS NSQIP — Covers a broad range of procedures with standardized risk-adjusted reports.
American Joint Replacement Registry (AJRR) — Tracks joint replacement outcomes including revision rates with risk adjustment applied.
CMS Care Compare — Hospital-level risk-adjusted readmission and complication rates for Medicare patients.

How to Use This Data

1. Evaluate programs first, then surgeons within programs. 2. Ask your surgeon directly: What is your program's STS rating or NSQIP performance? What is your O/E ratio? 3. Combine risk-adjusted data with volume data — a high-volume surgeon at a program with strong risk-adjusted outcomes is the strongest signal. 4. Be skeptical of raw rates presented without context.
